Texas Workforce Commission
  • The Texas Workforce Commission (TWC) has an intake questionnaire form to be completed when filing an employment discrimination complaint with TWC. The form is available on its website, http://www.twc.state.tx.us/crd/iq.pdf. You must contact TWC to set up an appointment to file the form. See their website at http://www.twc.state.tx.us/crd/file_emp.html for information on how to file a complaint.

    The site also has a lot of information on employment issues.
